We are seeking a Registered Nurse (RN) - Case Manager to assist our wounded warriors on their road to recovery!
Must have one of the following certifications: CCM, CDMS, CRRN, COHN, ACCC, CRC
Location: Peterson and/or Schriever Air Force Base in Colorado Springs, CO
Normal weekly work schedule is Monday-Friday 0715-1730; 8 hours/day; 40 hours/week
No holidays or weekends required!
No on call!

Provides case management, care coordination and discharge/disposition planning for inpatient and outpatient care settings.
Assists service members and/or veterans, family members and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.
Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in multidisciplinary patient care management practice.
Evaluates care and outcomes to ensure timely and appropriate provision of services.
Possess current certification in Basic Life Support (BLS) and Advanced Cardiac Life support (ACLS) by the American Heart Association.
Possess current valid nursing license from any US jurisdiction.
Be a graduate of an accredited National League of Nursing (NLN) school
One (1) year of case management experience
Be a US Citizen
